I am currently a fourth-year medical student going into internal medicine. When someone asks me, what books/resources did you use to survive medical school, several key resources come to mind: USMLE First Aid for Step 1 (aka the med school bible), pathoma, sketchy micro, onlinemeded, netters, and U-World. But now there is a new kid on the block... One that I can comfortably say with 100% confidence stands on par with these giants and is a must read any learner/aspiring healthcare provider… and that book is “Frameworks of Internal Medicine†by Dr. Mansoor.After, stating such a powerful claim, you are probably wondering how I can make such a statement? Let me explain:1. One of the hardest aspects of being a physician is creating a differential diagnosis. When you approach a medical student and ask them to list out a differential, let's say, pleural effusion, they immediately list out ten potential causes without a clear foundation in mind. Why is this a problem? Well to start, your list is likely to be incomplete without a basic structure. If I asked you the same question, but first asked you list all the causes of transudative pleural effusions followed by exudative pleural effusions, I guarantee your overall list will be longer. Your understanding of disease pathophysiology goes way up. If you listed nephrotic syndrome under a cause of transudative pleural effusion, it is easier to work out the physiology of pleural effusion secondary to nephrotic syndrome in your head if you already knew that it is a transudate process.2. Approaching each patient with a basic framework results in less room for error, less likely to miss a diagnosis, and a decrease anchoring bias. When you approach the lung exam, the first step is to inspect the lung, looking for asymmetry, deformities, etc. You do not just jump to auscultation. If you made that mistake, you might miss the possibility that a patient has a winged scapula, decreased breath movement on one side, etc. This is why having an approach in mind is necessary. You can even make the same argument for any procedure. Creating a differential diagnosis is no different. You are not only able to list more possible diagnoses, but also think through the physiology of each one. That is the beauty of frameworks.This book is superior in almost every way. This book presents an easy way to organize a differential surrounding common complaints and provides high yield key points in bullet and question format. I really wish I had this resource as a medical student to quickly find an organized approach to different chief complaints. Now, as someone who helps educate students on medicine wards, I would be incredibly impressed by a student who is able to think through a differential in this organized manner that promotes long term retention.For educators:This is an incredible resource to reference when creating "chalk talks" on the medicine wards. Students tend to find these brief, high-yield, learning sessions very enjoyable and helpful. Having these "frameworks" to draw out really make for organized and effective teaching sessions. As an added perk, the book comes with a code to be able to quickly reference the frameworks on a mobile app.Overall, this is a long overdue gift to the field of medicine and I foresee this book becoming a household name amongst medical trainees. I am a current medical resident, and purchased this book to help review and master common internal medicine topics. The book's approach is to the take the most common clinical presentations you will see on the wards and provide a structure to systematically approach each problem. As the title suggests, this "Frameworks" approach breaks each clinical presentation (e.g. "Chest Pain") into broad categories, then gradually builds increasing detail, in a structured approach that will help you learn as you read. In addition to visually presenting each framework as a decision tree, the text is written in a question-and-answer format that will force you to think through it as you go.The book is superb, and has improved my approach and understanding of topics that I have previously been exposed to many times. As a resident, you have seen much of this information before, but the book presents it in a clear and systematic way that will help you master it and apply in on the wards. I have read a thousand ABGs, but doing the acid-base section definitely improved my ability to read and interpret them more quickly and accurately on rounds.For medical students who are learning this for the first time, this book would be superior resource for learning and applying the material on board exams and on clinical clerkships. I would put it up there with Pathoma on my must purchase list of golden med school resources. A great read! I highly recommend!
